高雄榮總治療上顎口腔癌之臨床分析

Clinical Analysis of Surgical Treatment in Maxillary Malignancy from OMS, Kaohsiung-Veterans General Hospital

摘要

本報告針對高雄榮總口腔顎面外科自民國八十年迄今所有與上顎有關之口腔癌做一綜合整理及臨床分析，探討其年齡分佈、性別、嗜好、病灶位置、手術方法、重建方式、術後之再發率及存活率等相關資料，希望能從中增加對上顎口腔癌之瞭解、以便日後提供此類患者更好之治療方式。總計共收集到32個病例，其中上顎齒齦癌19例、軟腭癌9例及硬腭癌4例，腫瘤類型為鱗狀細胞癌26例、疣狀細胞癌4例、腺樣囊狀癌2例，25位男性、7位女性、男女生比例為3.6:1，年齡從36-87歲、平均為59歲，治療方式全部以手術為主及部份患者佐以術後放射線治療，手術方法大多採取廣泛性切除及部份上顎骨切除，而修補方式則以局部皮瓣重建為多數，結果發現術後病患在說話及吞嚥之功能上、有少數患者會有食物逆流至鼻腔之困擾，但大多數皆能適應良好，另外腫瘤之再發率為31.3%，概括存活率為75%，顯示此類病患比其他部位口腔癌患者之預後要好。

This study was to investigate the malignancies which involved maxillary gingiva, palatal mucosa and soft palate as the same entity from Oral and Maxillofacial surgery, Kaohsiung-Veterans Geneml Hospital during 1991 to 2003 for clinical analysis. The clinical data, including the age, sex, personal habit, tumor types and location, surgical methods and choice of reconstructive techniques were collected for evaluation. Totally 32 cases were selected with 26 cases of squamous cell carcinoma, 4 cases of verrucous carcinoma and 2 cases of adenoid cystic carcinoma for analysis. There were 25 males and 7 females patient with age from 36 to 87 years old. All patients were received surgical treatment with wide excision and some of them combined with alveolectomy or partial maxillectomy. Local flaps repair were the major reconstructed methods used in most patients. The result showed that most of the patient was well recovered with no significant disturbance of speaking and swallowing except a few patients had experience of nasal regurgitation. The overall survival rate was 75% and the recurrence rate was 3l.3%. Higher survival rate and better prognosis from other site of oral cancer were found from this kind of malignancies in our study.
