摘要

更年期的來臨主要是由於卵巢功能退化，致使月經週期逐漸延長，經量逐漸減少，直到完全停止；因卵巢內的濾泡數目逐漸減少，剩下一些品質較差的卵泡，女性荷爾蒙的量便隨之減少。據評估，停經以後，女性荷爾蒙主要成分之一的雌素二醇降低，不僅影響體內內分泌參與代謝調整功能，還會帶來一些不適症狀；此外，女性荷爾蒙不足，亦間接影響其他器官的內分泌功能，譬如腦下垂體負回饋機制所分泌的性腺激素－濾泡刺激素（FSH）跟黃體刺激素（LH）會慢慢升高，因此，抽血檢查血液中的雌素二醇（E2）和濾泡刺激素（FSH），可以用來判斷是否進入了更年期。中醫方面，對於更年期的定義，則是如《內經‧上古天真論》所云：「女子七七任脈虛、太衝脈衰少、天癸竭、地道不通，故形壞而無子也。」這是古人對絕經期的看法，認為49歲前後的婦女即面臨更年期。對於更年期的治療，則著重於調整內分泌以延緩機能退化，進而減輕不適症狀為主，較無副作用；例如：更年期所引起的骨質疏鬆症，傳統中醫以為腎主骨，故此即腎氣虛所造成，影響所及還包括泌尿、內分泌系統。

The oncoming of menopause is mainly caused by the degradation of ovary function, which lengthens the menstrual cycle gradually, and reduces its quantity until totally stopping. Because the number of follicles are reduced gradually in the ovary, the remnant follicles with worse quality reduce the amount of female hormone accordingly. It is assessed that the amount of Estradiol, one of major component of female hormone, which is reduced postmenopausal. not only influences endocrine secretion and metabolism adjustment function but also brings some malaise symptoms. In addition, if the female hormone is insufficient, it will also influence the endocrine function of other organs indirectly. For example, the amount of human gonadotropin secreted by negative feedback mechanism of hypophysis-Follicle Stimulating Hormone (FSH) and Luteinizing Hormone (LH)-will rise slowly. So we can determine whether someone is reaching menopause by checking the amount of Follicle Stimulating Hormone (FSH) and Luteinizing Hormone (LH) in the blood.As for Chinese Medicine, the definition of menopause is specified at the Doctrine of Ancient Natural Truth in the Internal Canon of Medicine,(”Nai Jing, Shang Ku Tien Chen Lun”), which is said “When woman is 49, it will be insufficiency in the Conception Vessel (Ren meridian), the Tai-Chong acupoint pulse will be fainted the Hypothalamus-Hypophysis-Thyroidal-Gonadal Axis (Tien-Gui) will be exhausted and the vagina would be dysfunctional, so woman' s body would be aging and become infertility” This is the viewpoint of ancients on menopause. It is considered that women around 49 years old face the menopause promptly. As for the treatment of menopause, it is focused on adjusting the endocrine to delay the degradation of function, and lighten the malaise symptoms with few side effects. For example, the osteoporosis caused by menopause is the insufficiency of kidney qi, because the kidney has close relationship to bone in the dialectical logic in Chinese Medicine, so it menopause can also influence the genitourinary system and the endocrinal system.
