井穴、合穴及原絡穴治療頸椎骨折導致癱瘓患者之病例報告

摘要

本病例為一頸椎骨折，導致截癱之患者。癱瘓（Paralysis）是指肌肉活動能力的減低或喪失，為神經系統常見症侯。按分布型式可分為偏癱（hemiparalysis）、截癱（paraplegia）和單癱（monoplegia）等。其中截癱即指兩下肢癱瘓。本患者在於頸椎骨折後，於外科加護病房中，其下肢肌力為0分，上肢肌力在給予大劑量類固醇及多巴胺後進步至3分，歸類為截癱之範疇。患者於加護病房住院期間，會診中醫治療，透過井穴、合穴及原絡穴等針刺療法，得以轉出加護病房，至一般病房繼續接受中西醫整合治療。對於針灸如何配合西醫重症治療及復健療程，以及井穴、合穴及原絡穴之功用及配穴思路，將於本病例中做進一步的探討。

This patient suffered from spinal cord injury with quadriplegia. During the hospitalization period, the neurosurgery doctor consulted Chinese medicine doctor to combine care. We treat him with acupuncture therapy. After one month treatment, he has good healing and can discharge from SICU to general ward for further rehabilitation therapy. In this report, we will describe this patient's treatment process and discuss about the indication and efficacy of these special acupuncture points.
