以張錫純「論火不歸原治法」治療陽氣浮越偏頭痛之病例報告

Applying Zhang Xichun's Theory about ＂Huo Bu Guei Yuan＂ to a Case of Recalcitrant Migraine

摘要

本醫案患者為39歲女性，八年前生產時因前置胎盤而大量出血，之後每次經來均有左耳外周頭痛的情形，且伴隨有高頻耳鳴。初診採用陰及和解少陽法未有明顯效果，後因注意到病患同時有下焦虛寒併氣虛的情形，加入少量溫陽藥以引火歸元，輔以參、赭之配伍，以參補氣，赭引參之補力下行、鎮而安之，結果使病患頭痛及耳鳴的情形均獲得明顯改善。

This case is a 39 year old woman; eight years ago, she had severe postpartum Hemorrhage, and after this event, she had been suffering from headache around her left ear accompanied by high-peached tinnitus a day or two before her every period. When she first visited my clinic, I tried to nourish her yin and mediate shao-yang. However, this strategy did not relive her symptom. During her second visiting, her inadequate in qui and yang was noted. Thus, I changed the direction of my therapy, added some yang-warming drugs to lead ＂yin huo guei yuan＂, and support their function with the companion of Renseng and Hematite in which the force of Renseng will be directed toward lower part of the body. As a result of this strategy, the patient's headache as well as tinnitus lessoned saliently.
