針灸治療膀胱過動症之病例報告

摘要

膀胱過動症為臨床常見的疾病，其涵蓋的範圍非常廣泛，包含急迫性尿失禁、混合性尿失禁以及一部分的應力性尿失禁。其定義為解尿急迫感、合併頻尿及夜尿，有些人可能伴有急迫性尿失禁。其評估方式包含病史詢問、理學檢查與尿液檢查，使用膀胱過動症調查問卷及排尿日誌。可用於評估病患於治療前後，症狀及療效的變化。其治療包含一般衛教、行為治療以及藥物治療，藥物治療通常選用作用於副交感神經藥物，用於減少膀胱逼尿肌收縮，但可能伴隨口乾舌燥、便秘等副作用。本病例報告為24歲膀胱過動症病患，因診斷膀胱過動症已九個月，期間也曾服用藥物但效果並不理想。故至本院中醫部尋求治療。在接受針灸治療後，各項症狀也迅速獲得良好控制，因國內期刊較少使用針灸治療膀胱過動症的病例，故整理此醫案作為針灸治療膀胱過動症的臨床參考。

Overactive bladder is a clinically common disease that covers a wide range of conditions, including urge urinary incontinence, mixed urinary incontinence, and part of stress urinary incontinence. It is defined as urgency, combined with frequent urination and nocturia, and some may be associated with urge incontinence. The assessment include history taking, physical examination and urine examination, use of the overactive bladder questionnaire and voiding diary. It can be used to assess changes in symptoms and outcomes before and after treatment. The treatment consists of patient education, behavioral treatment, and pharmacologic management. Drug therapy is usually used as a parasympathetic drug to reduce bladder detrusor contraction, but may be accompanied by side effects such as dry mouth and constipation. This case report is a 24-year-old overactive bladder patient who had been diagnosed of overactive bladder for nine months and received drugs, but the effect was not satisfactory. Therefore, he searched traditional Chinese medicine for help. After receiving acupuncture, the symptoms were quickly and well controlled. Because less acupuncture for overactive bladder was published in the journal, this case was arranged as a clinical reference for acupuncture of overactive bladder.
