全民健康保險自實施以來,醫療照護的可近性、費用支付制度、以及確保醫療品質等議題一直是備受討論的焦點。台灣地區評估醫療品質的方法,除了定期舉辦的醫院評鑑,尚有醫療院所進行的各式醫療品質管理活動。近年關於醫療品質的研究大多著重在醫療品質指標選定以及測量,對於醫療機構整體的醫療品質管理方向則較少著墨。因此本研究主要重點在於探討台灣地區各醫療院所實施療品質管理活動的現況,並進一步探討其實施動機與對醫療品質指標評估工具的看法,試提出促進國內醫療品質提昇之方案。 本研究採郵寄問卷方式,調查期間為民國八十七年八月至十一月初,調查對象為台灣地區健保特約醫院,共計回收130份。問卷內容主要為詢問該家醫院執行醫療品質管理活動的情況,以次數分配呈現資料分佈情形,並輔以邏輯迴歸分析醫院執行醫療品質管理活動以及實施成效的影響因子。
Since the implement f national health insurance, the discussion has been focused on issues such as the accessibility of medical care, the payment system, and the guarantee of the quality of medical treatment. Except for the hospital evaluation held regularly, the way to examine the medical quality in Taiwan also includes various activities of management conducted in medical institutions. Recent researches emphasize mostly on selection and survey of the index for medical quality, and it is less mentioned about the direction of management in medical institutions. Thus, this research stresses on the status quo of management activities of medical quality practiced in medical institutions in Taiwan, inquiring further the motives of practice as well as those ideas concerning the evaluation tools for medical quality index, then finally attempting to propose a scheme to raise the quality of national medical care. The method of this research is mailing questionnaires, which had investigated the medical service providers in Taiwan from August to November 1998. Totally there are 130 questionnaires reclaimed. The content includes the inquiries about the management activities the provider executes, and the execution and situation of related activities promoted by the Association of Medical Quality, R.O.C. in 1997.
