善用資通訊科技推動分級醫療之經驗分享

Experience in applying communication technology to the hierarchy of healthcare policy

摘要


目的:政府積極推動分級醫療與雙向轉診,留大病在大醫院治療,將小病轉至基層院所;本文將分享運用資通訊科技推動分級醫療之經驗。方法:以病人面臨的四大困境擬訂創新對策,由全人整合照護出發,建置智慧醫療資訊平台,鏈結醫院與社區醫療診所攜手合作的照護網絡,提供以家庭為基礎、以病人為中心的連續性整合醫療。結果:跨機關垂直/水平整合醫療暨雙向轉診資訊平台,提供醫院與社區醫療診所的醫師可即時查詢轉診病人就醫資訊,提升民眾轉診意願,達到衛福部2018年7月推動的大醫院門診減量2%之目標。結論:本專案運用智慧化整合資訊系統,簡化轉診作業流程,大幅提升醫師轉診的意願,成功達成門診病人減量的目標,備受衛福部的肯定,未來將持續精進分級醫療照護模式。

Purpose: The government has actively promoted classified levels of care and two-way referrals between domestic medical centers & medical care institutions intending to have patients with major diseases stay in major hospitals for treatment and patients with minor illnesses be transferred to primary health institutions. This article shares the experience of a medical center using information communication technology to promote classified levels of care. Methods: Based on the four major dilemmas faced by patients, we formulated innovative strategies for holistic integrated care first. The strategies involved building a smart medical information platform which was shared by our hospital and community clinics to provide information on family-based, patient-centric and continuous integrated medical care. Results: By cross-organization vertical / horizontal integration and a two-way referral information platform, we provided physicians in our hospital and community clinics real-time information about referred patients' care to increase doctors' and patients' willingness for referral, and achieved the goal of 2% deduction of the number of outpatients required by the Ministry of Health and Welfare in July 2018. Conclusion: This project used a smart integrated information system, simplified the referral process, significantly increased the willingness of doctors and patients for referrals, and successfully achieved the goal of reducing the amount of outpatients. Supported by the Ministry of Health and Welfare, We will continue to improve the graded medical care model in the future.
