Promoting Effective Communication Among Multidisciplinary Staffs by Using Information Technology to Transfer Patient Flow Information

摘要


「提升醫療照護人員間的有效溝通」是衛生福利部推動醫院醫療品質及病人安全年度工作目標之一;以病人為中心的醫療照護,涉及橫向的跨部門合作及縱向的連續性照護過程。本院自2007年起分階段建置資訊化輔助交班工具,以串聯各職系間照護模式,歷程包含「出院計畫評估及會診回覆系統」、「ISBAR電子交班平台」及「手術前/後單位間交接班平台」。有效地交班可確保病人安全和住院期間治療過程的連續性,本院病人安全通報事件中,歸因於「溝通不良」問題由2012年21件下降至2020年10件。運用病人流概念檢視本院電子交班資訊系統,透過資訊化輔助交班工具,採系統性作為方能有效改善落實交班、跨職系間的溝通與提升病人安全。

"Promoting effective communication between medical staff members" is one of the annual goals by Taiwan Ministry of Health and Welfare in promoting the quality of healthcare and patient safety. Patient-centered medical care involves interdepartmental cooperation and continuity of treatment process. Since 2007, we has established information technology tools to achieve complete handover in stages to connect the care models among multidisciplinary staffs. The information systems include "discharge planning evaluation and consultation system", "electronic handover platform of the ISBAR structure" and "preoperative /postoperative electronic handover platform". Effective handover is essential for ensuring patient safety and the continuity of treatment process during hospitalization. The cause of adverse events was reported as poor communication in 21 cases during 2012, which was reduced to 10 during 2020. We used the concept of patient flow to construct the electronic handover information system. The information technology tool has the advantage of synchronizing medical information between professionals systematically. It significantly improves completeness of handover, communication among multidisciplinary staffs and promotion of patient safety.
