Advances in the care of children with ARDS include lung-protective ventilator strategies, permissive hypercapnia, inhaled nitric oxide, high frequency ventilation and extracorporeal life support. These methods reduced ventilator-associated lung injury and may improve survival. Antenatal steroid, administration of surfactant reduced the mortality of prematurity. New modes of ventilator and liquid ventilation offer the potential salvage approaches for infants with respiratory failure. In this article, advances in pediatric respiratory care including protective mechanical ventilation strategy, high frequency oscillation ventilation, inhaled nitric oxide therapy, servo-controlled modes of ventilation, liquid ventilation will be discussed.
