Pulmonary hemorrhage is an unusual clinical manifestation of systemic lupus erythromatosus (SLE). Fulminant pulmonary hemorrhage as the initial manifestation of SLE in pediatric patients is rare and the mortality rate is high. We report a 16 year-old female who initially presented with fulminant pulmonary hemorrhage; she was diagnosed as having SLE and was successfully treated by early aggressive management with methylprednisolone and cyclophosphamide pulse therapies. It is important to keep in mind that pulmonary hemorrhage can be the initial presentation of SLE. Early diagnosis and rapid intensive care arc necessary to successfully rescue SLE patients with fulminant pulmonary hemorrhage.
