Complications Associated with Peripherally Inserted Central Venous Catheter in Neonatal Intensive Care Unit
周邊經皮置入中央靜脈導管在新生兒加護病房所發生的併發症
摘要
目的：以回朔的的方式分析在一年之內因周邊經皮置入中央靜脈導管所產生的併發症，比較在更換廠牌之後產生的併發症，並試圖分析產生併發症的原因。 材料：在2005年7月份更換導管品牌(Bard, Per-Q plus 2Fr. Single-Lumen PICC)，同年八月，發生4例併發症，其中一名新生兒死亡。 結果：案例一，出生體重1308公克，在導管置入21天後在右臂及手指產生嚴重的皮下水腫。案例二，出生體重920公克在導管置入37天後產生上腔靜脈完全阻塞，右側嚴重積水（乳糜胸），胸壁及手臂鍾脹，疑似上腔靜脈症候群。案例三出生體重1004公克在導管置入26天後產生右側乳糜胸。案例四出生體重1009公克在導管置入隔天產生靜脈炎。 由置放導管置併發症產生的天數平均為28天並且在併發症發生時沒有菌血症被發現。所選擇置放導管的靜脈有三個一樣皆由肘窩靜脈下針，有一名由大隱靜脈下針，導管末端所在的位置都在中央靜脈。 結論：儘管在美國也是使用此款導管，不過併發症發生率並沒有我們著兩個月的這麼高。比較了兩種導管(Vygon, Bard)，材質皆為矽膠，大小皆選擇一樣，不過細項分析的流速重力(flow rate gravity)是不一樣的。此外，在更換回原本的品牌之後，迄今並沒有類似的併發症產生。

Objective: To retrospectively evaluate the factors resulting in the complications of consecutively inserted percutaneous central venous catheters (PCVC, silicone, Bard) in newborns over a 1-year period. Patients' population: Four serious complications were noted after change the brand of PCVC. All cases received the same silicone PCVC (Bard, Per-Q plus 2Fr. Single-Lumen PICC). Case 1 (birth weight 1308g) perforated PCVC caused subcutaneous edema over right arm and fingers 21 days after the insertion of silicone PCVC. Case 2 (birth weight 920g) developed superior vena cava (SVC) collapse, SVC syndrome, and right lung chylothorax with neck and chest wall swelling on the 37(superscript th) day of silicone PCVC-insertion. Case 3 (birth weight 1004g) got right pleural effusion and chylothorax after perforation of the same PCVC into the right pleural cavity on the 26(superscript th) day of PCVC-insertion. Case 4 (birth weight 1009g) suffered from left lower legs phlebitis after secondary day of the same PCVC-insertion. Results: The data showed that PCVC-associated symptomatic perforation complications occurred after the use of median 28 consecutive days (21~37 days) regardless of the status of bacteremia and acidosis.
