Clinical Applications of Therapeutic Plasma Exchange in Kidney Diseases
摘要
治療性血漿置換術(therapeutic plasma exchange)可以去除致病性的大分子物質，例如自身抗體，異體抗體，抗原抗體複合物等，是一種安全有效的血液淨化治療。應用在腎臟疾病的適應症包括：快速進行性腎絲球腎炎、器官移植、血栓性血小板低下性紫斑症、局部節段性腎絲球硬化症移植後復發等。建議可依照實證醫學的證據等級選擇適合血漿置換的病人，並配合免疫抑制劑和其他血液淨化方式，才能發揮治療性血漿置換術的最大效益。

Therapeutic plasma exchange is a safe and effective method of blood purification. It can remove large pathogenic molecules such as autoantibody, alloantibody and antibody-antigen complex. Indications for plasmapheresis in nephrology include rapidly progressive glomerulonephritis, organ transplantation, thrombotic thrombocytopenia purpura, and recurrent focal segmental glomerulosclerosis in kidney graft. Following the recommendation of current best evidence to select patients that are suitable for plasmapheresis is suggested. Combination therapy with other immunosuppressive agents and blood purification modalities is important to maximize the effects of therapeutic plasma exchange.
