摘要

糖尿病是全世界流行的慢性病，控制不良會導致心血管及腎臟的損傷，嚴重將造成慢性腎臟衰竭。國外研究顯示，不論是第1 型或第2 型糖尿病所導致的腎病變，接受腎臟移植都有較高的存活率，但於移植前、後有其特別的考量。提供有效的術前評估與協助，對糖尿病患者的腎臟移植是重要的一環。故本文簡介糖尿病末期腎病患者之術前篩檢與評估、術後藥物使用以及血糖控制。希望透過有效的篩檢與照護衛教，提高糖尿病腎臟移植患者的生活品質與存活率。

Diabetes is a worldwide epidemic disease. It may lead to damages of the heart, blood vessels and kidney. Poor control of diabetes may cause serious chronic kidney injury. In addition, researches also reported both type 1 and type 2 diabetes will achieve higher survival rate after receiving kidney transplant. But, there are some special issues should be considered in peri-transplantation stage. Therefore, timely assessment and proper implementation could help diabetes patients in this stage. This paper aimed to review screening process of patients with diabetic end stage renal disease, preoperative assessments, and postoperative transplant medications and glycemic control methods. We hope effective screening and proper perioperative care could lead to better life quality and long term survival of diabetic transplant patients.
