There is a large and growing representation of older candidates on the kidney transplant waiting list. More recently, efforts to meet the growing demand for transplantation have included the increased use of older donor organs which has similar outcomes to older recipients of younger donor kidneys. The expanded criteria donor policy may decrease the waiting time for kidney transplantation in older recipients. However, the changes in allocation policies in the USA has led to new controversies and challenges. Transplant physicians are now faced with rationing a limited resource and maximizing long-term outcomes for recipients of all ages. Future studies should explore which donor and recipient pairs will achieve the best long-term outcomes.
