Chronic kidney disease (CKD) is recognized as an independent risk factor for acute kidney injury (AKI). AKI is an independent risk factor for de novo CKD. AKI also accelerates the progression of CKD. The spectrum of disease state and outcome is associated with the balance of adaptive and maladaptive repair mechanisms to renal insults. We need to identify AKI as a major risk factor for adverse outcome, and provide long-term follow-up to patients with episodes of AKI. Besides, we also need to set up strategies for management of affected patients, and develop therapeutic options to prevent progression from AKI to CKD.
