In this article, we shared our experience to show how a peritoneal dialysis nurse cares for a patient with encapsulating peritoneal sclerosis when shifting the dialysis modality. During the period of medical care, we collected four domains of information including physiology, psychology, society, and spirituality from this patient. The problems of malnutrition, vascular access failure, and uncertainty in illness were first identified. Then we designed a series of care plans to improve her physical and psychological condition, quality of life, and reduce uncertainty in illness, including nutrition and disease education programs, technics for shunt care, resistance exercise, and listening and communicating. Through these procedures, we successfully convinced the patient to keep on hemodialysis with a positive attitude rather than fear.
