牙周及植牙周圍組織的生物觀

摘要


人工植牙對牙科有革命性的影響,過去幾十年來,有許多植牙系統不斷問市,但並非所有的產品都有嚴謹的動物實驗報告及足夠的成功臨床經驗。我們必須根據牙齒及人工牙根在口腔環境中生物性的考量來選擇一種適合患者的植牙系統或特殊表面處理的人工牙根。成功的植牙及合乎美觀的結果須要成功的硬組織及軟組識處理,這端賴於對自然牙的牙周組織及人工植牙的周圍組織的生物性及解刻構造有更深的了解。而造成植牙失敗的生物性因素包括的內在因素有:患者健康狀況,吸菸,不良骨質、量,咬合異常功能;外在因素有:手術者的經驗,手術時的傷害,細菌感染,不好的人工牙根表面性質或不理想的植牙設計。若能對人工牙根的生物構造有更深一層的了解,可以減少植牙周圍發炎的機會;得以在植牙周圍軟組織及硬組織的健康及美觀之間求取一平衡。

Oral implants have a revolutionary effect on the practice of dentistry. Over the years, a large number of different implant systems have been introduced. But not all of the products were supported by controlled animal studies and enough successful clinical experiences. We have to choose a patient- oriented dental implant system or specific surface managed implant based on the biological considerations of teeth and implants in the environment of oral cavity. The successful management of hard tissue and soft tissue resulted in esthetic and functional outcomes; all of these depend on profound understanding of the biological characteristics and anatomic structures of the periodontal tissues and pen-implant tissues. The biological factors associated with implant failures include endogenous: medical status of the patient, smoking, bone quality and quantity, parafunctions; exogenous: operator experience, surgical trauma, bacterial contamination, non-optimal surface properties, and non-optimal implant design. In order to reduce the chance of peri-implantitis, practitioners should understand the biological basis of oral implants. Thus, pen-implant tissues can express a balance between health and esthetics.
