牙周炎與T淋巴球及樹突狀細胞之相關性-文獻回顧

The Association of T Lymphocytes and Dendritic Cells with Periodontal Disease: Review of Literatures

摘要


淋巴球免疫反應在牙周破壞中佔有重要的地位,在牙周炎局部病灶區內含有大量的淋巴球細胞激素,觀察病患不同時期病灶組織,會發現浸潤淋巴球種類有改變的現象,而T淋巴球被認為對於調節後期免疫反應佔有關鍵的角色。1990年初期,許多重要的牙周病破壞性介質,如:IL-1、PGE2、及metalloproteinase等作用已經確定,但其出現與調節理論,目前仍無法完全定論,同時對於T淋巴球反應的型式,與牙周疾病進程的關係,目前仍處於爭議的狀態,因此後來的研究著重於探討初期免疫對繼續發性免疫反應的影響。樹突狀細胞,爲體內有效率的抗原呈現細胞,它能藉由分泌發炎前的細胞激素,來調控T淋巴球的行爲。在牙周致病菌入侵時,是否樹突狀細胞接觸抗原後,能藉由初期免疫引導後期淋巴球産生Th1/Th2的表現,引導牙周疾病産生不同的結果,仍是未來研究的方向。

The immune response of the lymphocytes plays an important role in periodontal. High amounts of lymphokines can be detected in the local periodontitis lesions. As periodontal destruction progresses, a shift from T lymphocytes to B lymphocytes and plasma cells was observed. The T cells are considered a key factor to regulating the immune response. But the expression and regulation of adaptive immune responses in periodontal disease progression remains controversial. In recent studies, attentions focused on the effects of innate immune response. When dendritic cells (DCs), the most effective antigen-presenting cells, capture and process antigens, they will mature and secrete cytokines to regulate T cell immune response. When contact with the periodontal pathogens, whether or not the dendritic cells can modulate the adaptive immune response to regulate the disease progression still need further study to be determined.
