Allergy Does Not Influence the Severity of Chronic Rhinosinusitis

摘要

目的 過敏和慢性鼻炎鼻竇炎的相關性，一直是尚未定論的話題。本研究的目的在瞭解過敏與慢性鼻炎鼻竇炎嚴重程度的相關性。
方法 本研究收集自2000年3月至2002年6月間，診斷為慢性鼻炎鼻竇炎且接受鼻竇內視鏡手術的病人共163名。所有病人均接受皮內皮膚試驗及電腦斷層檢查。比較有無過敏兩組病人術前鼻竇電腦斷層，問卷分數，聲反射鼻量計檢查及嗅覺功能檢查的結果。另分析病人填寫之問卷分數和鼻竇電腦斷層嚴重程度之相關性。
結果 兩組病人術前鼻竇電腦斷層，問卷分數，聲反射鼻量計檢查，及嗅覺功能檢查的結果在統計上沒有明顯的差異。病人填寫之問卷分數和鼻竇電腦斷層嚴重度有顯著相關性。
結論 雖然過敏為慢性鼻炎鼻竇炎可能的病因，患者未因過敏的存在而影響鼻炎鼻竇炎的嚴重度，鼻腔通暢度及嗅覺功能。然而過敏的控制對於慢性鼻炎鼻竇炎的治療還是十分重要。

Objectives. The relationship between allergy and chronic rhinosinusitis has not been clearly defined. The goal of this study is to investigate whether there is any correlation between allergy and the severity of sinus disease.
Methods. We studied 163 patients with chronic rhinosinusitis. All underwent an intradermal skin test and coronal sinus computed tomography (CT scan). The results of a questionnaire, scoring, smell thresholds, and acoustic rhinometry were analyzed. The correlation between sinus CT scores and questionnaire scores was also analyzed.
Results. There were no significant differences in CT scores, questionnaire scores, smell thresholds and the second minimal cross-sectional areas from acoustic rhinometry between allergic and non-allergic patients. The CT scores and questionnaire scores showed strong correlation.
Conclusions. Allergy does not influence the severity of chronic rhinosinusitis. However, allergy control is important in the treatment of chronic rhinosinusitis.
