景岳全書婦人規中有關月經不調之研究
The Study of Irregular Menstruation In the Fu Ren Gui of the Jing Yeu Quan Shu
摘要
本文的目標是在探討月經不調辨證方式與用藥概念，所用的方法是將《景岳全書．婦人規》有關月經不調的章節，依症狀、診斷、治療、用藥，進行分類、歸納與分析。結果是景岳對於月經不調辨證，經早以血熱為主；經遲區分為血熱和血寒；經亂區分為血虛和腎虛。而用藥只要是月經不調則偏好熟地。結論為診治月經不調時，建議以八鋼辨證為主，輔之以月經辨證。對於月經不調的治療用藥宜有陰陽互根的觀念。由於現代女性大致上的體質偏向血熱，研究中發現景岳的用藥思想恰好十分適合現代婦女的月經不調。另外亦發現古籍中有關醫案病例的記載如果能加上流行病學的概念的「人、事、時、地」的整體觀察及描述，應該能使我們後世對於月經不調的致因及療效更容易掌握。

Objective The paper major conferred the method of the pattern identification and the concept of using medication in irregular menstruation. Material and Method: We chose Jing Yue Quan Shu about Fu Ren Gui as our material. We were proceeding to class, generalize and analyse the data by means of symptom, diagnosis, treatment and medication according to the ancient theory. Result : According to Fu Ren Gui about the method of the pattern identification in irregular menstruation. Advanced menstruation was mainly blood heat. Delayed menstruation differentiated between blood heat and blood clod. Chaotic menstruation differentiated between blood vacuity and kidney vacuity. Jing-Yue Zhang used to medication in irregular menstruation, preferred choosing cooked rehmannia. Conclusion: We suggested that the method of the pattern identification first chose eight-principle pattern identification and menstrual pattern identification was secondary. As regards medication, it was necessary that the concept of yin and yang were rooted in each other. Owing to the constitution of women of the present generation more likely to be blood heat, we found that about the thought of Jing-Yue Zhang used to medication in irregular menstruation agreed with women of the present generation. Besides, we found that ancient books about casereport recording were lack of the concept of epidemiology. e recommended that Traditional Chinese medicine doctors in Taiwan need to describe”who, what, when, where”in case report more detail in present practice and to conduct large scale trial to confirm the safety and efficacy of this kind of treatment. So we would integrate traditional theory into main stream medicine in irregular menstruation that might generate new hypothesis in treatment.
