中醫介入原發性痛經之病例報告

此23歲女性因每月痛經不適而前往高醫中醫部就診，病患自訴從上高中以來每次經期來皆會下腹疼痛，經來第一、二天疼痛分數約5-6分，會疼痛致嘔吐，每次都需服止痛藥（一天兩顆），月經規則，經色鮮紅量正常，經前第一、二天常有約拇指般大血塊排出，且在排卵期及來經前雙側乳房脹痛不適。自訴平素因工作壓力大易情緒緊張，而情志緊張屬肝，肝臟因情志不暢，鬱怒所傷，失於疏泄所致氣機鬱滯。經中醫四診合參，開藥診治後，下個月痛經情況得到改善，但因此為治標，如何治本讓患者痛經症狀一勞永逸解除，需搭配後續治療外，飲食與生活作息衛教也是不可忽缺。

This 23-year-old woman went to the department of Traditional Chinese Medicine atKaohsiung Medical University Chung-Ho Memorial Hospital(KMUH) for helpbecause of monthly dysmenorrhea. The patient complained that since she was in high school, her lower abdomen was painful every time during menstruation, and the pain score could reach about 5-6 on the first and second days of menstruation. Sometimes it even caused vomiting.She needed to take painkillers (two pills a day) during menstruation for this problem. The menstrual cycle is regular, and blood clots about the size of a thumb are often discharged on the first and second days before menstruation. Moreover, both breasts are painful and uncomfortable during ovulation and before menstruation. She claimed that she was easily emotionally stressed due to working pressure, and emotional stress was related to the liver. The liver was injured by stagnation of emotions and anger. Afterfour diagnostic methods of traditional Chinese medicine,the dysmenorrhea condition was improved in the next month. However, for the symptoms, how to cure the patient's dysmenorrhea symptoms once and for all, it needs to be combined with follow-up treatment, diet and life, health education is also indispensable .
