不孕症促排卵西藥反覆治療後之中醫治療病例報告

Infertility Treated by Traditional Chinese Medicine after Repeated Treatment of Ovulation Stimulants: A Case Report

摘要

本病例描述一位35歲女性教師，G0P0A0，結婚逾1年仍未懷孕。經婦產科檢查無特殊解剖與生理上問題，病患接受西藥排卵藥Clomiphene治療數回仍無法成功懷孕。病患就診前月經偏向後期，基礎體溫呈鋸齒狀，伴隨子宮內膜過薄、淺眠多夢、夜間頻尿，大便稍軟且不成形等症狀，辨證為腎虛、脾虛與心血心陰不足，治療依循中醫月經週期調經法之原則，採取益氣健脾，滋補腎陰，養血安神為主之治法與用藥，治療後基礎體溫呈現雙相且溫度較平穩，最後成功懷孕，妊娠期間一切諸症穩定，患者預產期預計為6月16日。本案例我們討論了中醫在配合西藥排卵藥治療的思路與想法並探討了心（腦）—腎—子宮生殖軸學說。

This case describes a 35-year-old female teacher, G0P0A0, who has been married for more than 1 year and has not yet become pregnant. After gynecological examination, there were no special anatomical and physiological problems, and the patient was unable to conceive after several times of treatment with the ovulation stimulants Clomiphene. Before visiting the doctor, the menstruation tends to be late, the basal body temperature is jagged, accompanied by symptoms such as thin endometrium, light sleep and dreaminess, frequent urination at night, slightly soft and shapeless stools, etc. Replenishing qi and invigorating the spleen, nourishing kidney yin, nourishing blood and calming the nerves are the main treatment methods and medications. After the treatment, the basal body temperature is biphasic and relatively stable. Finally, she successfully conceived. The expected delivery date of the patient is June 16. In this case, we discussed the thoughts and ideas of traditional Chinese medicine combined with ovulation stimulants and discussed the theory of heart (brain)-kidney-uterine reproductive axis.
