摘要


肩部承接軀幹與上肢的結構,也負擔著人體正常活動功能的重擔,舉凡各種推、舉、拿、取、、等。這些動作皆須透過正常的肩部構造來加以執行。因此,肩部的正常與否,密切的影響著人體日常生活機能。一旦發生狀況,將對患者造成極大的痛苦和不便。盂肱關節的前、後脫位分離,為肩部創傷所常見。對於這個重要結構,臨床上有不同的檢查選項,其中最為迅速,也可提供初步診治資料的,是採用X-射線進行「肩部投射」攝影成像檢查。本文以分析比較兩種肩部X射線成像檢查技術,即「X射線之肩部前後斜位投射攝影(Anterior Posterior Oblique Projection-Glenoid cavity)(Grashey Method)」和「X射線之肩部前後頂端斜位軸向投射攝影(Anterior Posterior Apical Oblique Axial Projection: Shoulder)(Garth Method)」在盂肱關節前、後脫位的應用檢查中,「肩部前後頂端斜位軸向投射攝影(Garth method)」成效更佳的因素。讓醫事放射師在技術操作以及影像呈現上,有更為專業判斷依據。

The shoulders not only connect the trunk region and the upper limbs but also are in charge of many movements in our life. Both the Garth method and the Grashey method can clinically display the structure of the glenohumeral joints. It is very important for radiological students and technicians to master the Garth method and the Grashey method. First, this article describes the basic dissection trend of the glenohumeral joints. Second, it explains the techniques of these two methods briefly. And last but not least, we compare the images features of these two methods for detection of dislocation of the glenohumeral joints. According to these analytic results, it demonstrates that the Garth method is more valuable than the Grashey method in the diagnosis of anterior and posterior dislocation of the glenohumeral joint.
