摘要
牙周病的病因雖然目前仍不能完全了解,但是最近從許多的動物及人類的研究報告,顯示牙菌膜的存在,進成牙周組織病蠻的因果關係是無庸置疑的。因此到目前為止,牙菌膜仰然被器為是牙周病最重要的致病因素。大體而言,牙周病的病因,大致可以歸納第三種。1.主要因素: 最主要的致病因素;包括牙菌膜及其代謝作用的產物如內毒素(endotoxins) ,外毒素(exotoxïns) ,酵素( enzymes)及兔疫病理機轉(immunopathic mechanisms)的綜合作用。2。局部因素( local factors) :包括牙醫師的人為因素(iatrogenic factors),牙結石(calculus) ,咬合性傷害(traumatic injury) ,化學藥 品的傷害(chemical inj ury) ,牙齒及牙根形態的異常和過度的咬合壓力(excessive occlusal force) ...........等。3.全身性因素( systemic factors) :包括內分泌功能障礙(endocrine imbalance)、血液性疾病(hematologic disorders)、神經系統的影響(neurous system influences)、藥物後遺症(drug manifestations)‘營養不良( malnutrition)、遺傳性疾病( hereditary di seas es)和自血球功能障礙(leukocyte dysfunction)等。

The extentive population studies regarding the etiology, development and prevention of periodontal disease have been derived mainly from reports of clinical trials in human and animals. All of these reports indicated the role of microbial dental plaque as the major etiological factor in the initiation and ?evelopment of gingivitis and periodontitis. In general, the etiologic factors of periodontal disease can be summarized as follows: 1. primary factors such as microbial dental plaque, bacterial products, bacterial enzymes, endotoxin and chemotactic factors; 2. local factors including iatrogenic factors, calculus, traumatic injury, chemical injury, tooth position, root anatomy and excessive occlusal force; 3. systemic factors consist of endoccnne imbalance, hematologic disorders, neurous system influences, drug manifestations, malnutrition, hereditary diseases and leukocyte dysfunction etc. In addition, the docu:ne~tation of the validity of the hypothesis of as well as nonspecific microorganisms in gmglvitis provides effective keypoints of development of methods for the control and prevention of the inflammatory periodontal disease.
