實施「安寧緩和醫療條例教育訓練方案」成效之初探

The Study for the Effect of Natural Death Act Educational Program

摘要


本研究目的是探討護理人員對安寧緩和醫療條例知識程度,以及執行教育訓練方案對安寧緩和醫療條例知識程度之差異。本研究採類實驗性研究設計,針對某一所醫學中心志願參加教育訓練方案之護理人員以安寧緩和醫療條例知識量表及基本資料表,進行問卷調查,共發出135份問卷,回收105份,回收率為85.37%。教育訓練方案之時數為6小時,內容包括安寧緩和醫療、安寧緩和醫療條例、如何幫助面對苦難的病人與家屬、安寧護理的臨床實務及瀕死病患的照顧等五個主題,授課方式結合講授、案例分享、檢視個人價值觀及面對死亡病人的態度。研究結果:(1)執行教育訓練方案前,在18題安寧緩和醫療條例知識量表中,答對率由40.6%至100%,平均答對率為72.09%,(2)執行教育訓方案後,安寧緩和醫療條例知識量表之平均總得分由12.9分增加至17.01分(總分18分),提昇了4.08分,達到統計上顯著差異。本研究結果建議安寧緩和條例教育訓練應在護理學生的養成教育中納入,也是新進護理人員與在職護理人員教育訓練的重點,未來更應朝著符合病人期待及倫理原則的醫療決策做努力。

This pilot study aimed to find out the knowledge level to natural death act for nursing staff, and the differences of understanding after educational training program of natural death act. It is a quasi–experimental designed study. 135 copies of questionnaires, including natural death act knowledge items and personal background, were given to the nursing staffs of a medical center who volunteered to join the educational training program. 105 subjects were recruited. The response rate was 85.37%. 6-housr educational training program contained five topics: hospice palliative medicine, natural death act, how to accompany terminal patients and their families, and hospice clinic nursing care and pre-dying patient care. Lecture, case study, and review of personal value and attitude of facing death were used. The results indicated that : (1)for the natural death act 18 items, the average correct ratio is 72.09% before educational program, (2). After educational training program, the average score was improved from 12.9 to 17.04 (18 is full grade). This result showed that natural death act should be included in nursing school educational program. The program was also the key point for new comers and working staff. In the future, more effort should be made to fulfill patients’ expectation and to follow the guidelines of medical ethic.
