摘要

安寧共同照顧的發展是為了打破了病人觀念、照顧場所等因素之限制，將安寧照顧送到有照顧需求地方；亦即將安寧四全照顧之精神運用在尚未轉入傳統安寧照顧方式的急性病房中的末期病患。其運作是以分擔照顧責任(shared care)之方式進行，此一方式與傳統的“照會－會診”方式之不同點在於隨著病患之症狀與需求，機動調整照顧責任之分擔，安寧共同照顧團隊之角色會有提供建議、示範照顧方式乃至暫時承擔部份照顧責任之變化。因此，安寧共同照顧團隊兼具提供照顧與教育急性病房醫療團隊之雙重任務。要能符合上述任務之要求，安寧照顧團隊之基本成員最好是具有安寧病房照顧經驗的資深安寧緩和醫療專科醫師與護理師，並視業務推展佐以社工師。安寧共同照顧之業務推展成功的要素，有賴醫院正視末期病患有綜合了生理、心理、社會及靈性的照顧需求，並將滿足此需求視為當然之責任，繼而宣導各病房轉介病患並參與安寧共同照顧，使得提供符合末期病患需求之照顧成為全院醫療品質的核心指標。

The development of hospital-based palliative care team to provide hospice shared care breaks the limitation set by misunderstanding of hospice care and sends hospice care to where the patients need it. That is, providing hospice-palliative care in acute hospital setting. The hospice-palliative care team and the acute ward team work together to share the care responsibility. According to the problems and needs presented, hospice-palliative team may provide suggestion, or demonstrate specific technique, or take over part of the responsibility temporarily. So, the hospice-palliative care team has the both roles of providing care and educating the acute ward team-members. To play the both roles, it is better that the team is consisted of senior physician and nurse who have the experience of in-patient hospice care, as well as social worker if the work loading increases. The successful development of hospice share care depends on the commission of hospital to provide care which meets the bio-psycho-social and spiritual needs of terminal patients. So that colleagues of every ward in the hospital are willing to consult the palliative care team and share the care responsibility.
