協助病人善終專案

Development of End-of-life Care in Achieving Good Death

摘要


本院緩和療護病房於92年9月成立,由督導長,召集護理長及護理人員8位,組成安寧病房籌備小組,建立完整且符合安寧療護理念的作業標準。團隊參考實證醫學,建立護理常規標準和特殊紀錄單張並擬定教育計畫。利用「善終評核表」及「善終知識自我準備度」作為測試工具,前測為籌備團體剛成立時,後測為開始服務後1個半月。「善終評核表」滿分為90分,前測平均為23分,目標設定為56分,後測平均為62分。「善終知識自我準備度」滿分為20分,前測平均為7.5分,目標設定為14分,後測平均為14分。專案實施結果,確實讓同仁儘快能熟悉緩和療護病房的照顧模式及運作方法。也按月舉辦往生個案討論以檢討照顧過程;依同仁需要舉辦相關在職教育,增進病人及家屬福祉。

Our hospital starts hospice in-patient care since 2003, Sept 1. Before the palliative care unit started clinical service, a team of nursing staff was organized and trained in the preparation period. The team consists of 8 nurses with nursing experience ranging from 5 to 14 years. The nurses completed a ”Checklist for good end-of-life care” (total score 90) and a ”Level of readiness of knowledge and attitude of good end-of-life care” questionnaire (total score 20) the week before and 2 months after the start of service. The standards of nursing routines and the special charts for these routines were developed. An education plan was also developed with special focus on the end-of-life care. In the pre-test, the average score of ”checklist for good end-of-life care” was 23 and the average score of ”Level of readiness of knowledge and attitude of good end-of-life care” questionnaire was 7.5. After the integrated service was actually provided, the average score of ”checklist for good end-of-life care” was 62 and the average score of ”Level of readiness of knowledge and attitude of good end-of-life care” questionnaire was 14. Through the development of relevant routines in the form of special chart, it helps the team members to deploy the routines and the usage of educational resources, and thereafter improve the ability to provide end-of-life care.
