住院中低氧合病患使用侵襲性正壓呼吸器的成效

Outcome of in-patient with Poor Oxygenation Using Invasive Positive Mechanical Ventilator

摘要


We conducted this study to discuss about the poor oxygenation patients requiring invasive positive mechanical ventilation (MV)and PEEP, including: 1) Outcomes in different reason of using MV, 2) the treatment result in various patients of poor oxygenation using MV and PEEP, 3) the predictors of weaning rate and survival rate during MV and PEEP use. Poor oxygenation was assigned by the following criteria; FIO2>50%、PaO2/FiO2≦200mmHg. A total of 638 patients with MV and PEEP were recruited in our hospital from January to December 2007. The rate of male and female were 483/155, the mean age was 67±16 years old, the successful weaning rate and survival rate was 25.9% and 37.8% respectively. The most frequent using ventilator factor was infection (45.1%), which pneumonia was 182 patients (28.5%). Comparing the various maximum PEEP of poor oxygenation groups during ventilator use (PEEP:0-5cmH2O、6-10cmH2O、11-15 cmH2O、>15 cmH2O), The higher PEEP group compressed with other groups was statistical significance increase MV days (17±19、22±23、21±19、26±21), and also statistical significance lower both survival rate (50.9%、38.4%、28.4%、22.5%) and weaning rate. (37.2%、25.0%、17.7%、15.0%).
