重度肥胖合併急性呼吸衰竭病人之呼吸照護

Respiratory care for patients with morbid obesity and acute respiratory failure

摘要


肥胖所延伸的問題與嚴重度在過去數十年來逐漸攀升,相關併發症的治療費用也有增無減。世界肥胖聯盟(world obesity federation)估計,在西元2025年前,全球過重或肥胖人口數會從2014年的20億人增加到27億人,研究證實肥胖會引起許多共病症,且經常合併呼吸問題,從輕微呼吸急促到可能危及生命的呼吸衰竭。肥胖儼然已是重要的公共衛生問題,與生活息息相關。本個案為重度肥胖合併肥胖換氣不足症候群的急性呼吸衰竭病人,經插管給予呼吸支持,待生命徵象穩定及拔管評估通過後,予以拔除氣管內管使用非侵襲性呼吸器的病人,藉此個案的呼吸照護過程,探討肥胖對呼吸系統的影響,並提供臨床人員此類病人之照護經驗。

The problems and severity of obesity have gradually increased over the past few decades, and the cost of treatment for related complications has increased. The World Obesity Federation estimates that the number of overweight or obese adults in the world will increase from 2 billion in 2014 to 2.7 billion by 2025. Obesity is already an important public health issue and is closely related to life. Studies have shown that obesity causes many comorbidities. Obese people usually have respiratory problems like dyspnea and respiratory failure. This case report discussed about the morbid obesity patient who has acute respiratory failure combined with obesity hypoventilation syndrome. To support respiratory system, the patient was intubated. After the vital signs were stabilized and the weaning profile was passed, the endotracheal tube was removed and the non-invasive ventilator was used. Therefore, the respiratory care process in this case is used to explore the effects of obesity on the respiratory system and to provide clinical staff with experience in care of such patients.
