摘要

精神醫療機構中，住院病人團體心理治療之場域較為特殊，包括住院時間短、流動率大、異質性高、團體界線模糊等特質，這些對於團體心理治療帶領者而言，有著較大的挑戰。因此，本文將回顧住院病人之團體心理治療模式，以瞭解病房團體之特性、團體目標、以及團體帶領者的角色與任務；並結合實務經驗，呈現目前精神官能症病房人際主動團體之運作方式、團體挑戰，以及可能的因應方式；更進一步，試圖將心理動力取向之概念融入團體之中，包含心智化、偏執－分裂心理位置、憂鬱心理位置以及團體基本假定，以提昇對病人內在心理狀態之理解。

The aim of this study was to present the features of interactional group therapy in a neurotic ward. Firstly, we tried to review the working model and the characteristics of inpatient group therapy including the role of the group leader. Secondly, several challenges of inpatient group therapy in a neurotic ward were revealed and discussed. Thirdly, we applied some psychodynamic concepts, including mentalizing, paranoid-schizoid position and depressive position, and the basic assumptions of groups, into the practice of group therapy, and some possible interventions were offered and further discussed.
