封閉式中文聲母語詞聽閾檢定表的編制

Consonant Words Lists for Consonant Discrimination Test in Mandarin speech Audiometry

摘要


本文應用電腦統計國民小學國語課本第一冊至第十二冊的語詞與字彙,依出現率建立詞庫資料,將詞庫中依聲母語詞別(即子音別)有重覆出現率達35次以上之詞彙收集做內容的編排,先以標準化的錄製過程,並依聲母別的語音均衡原理,使用電腦編輯成四份檢定表共80題,並以15名30耳正常聽力的年輕人做測試,證實其可靠度與效度良好,與言語收聽閾值(speech reception threshould,簡稱SRT)測驗比較相關性亦高;但之後統計分析發現,四份檢定表的難易度不均且同一份檢定表內的成績相關性過高,經統計集群分析重新選題,仍依聲母別之語音均衡及相異性原則編成三份新的封閉式檢定表,再經信度分析及與SRT測驗相關性比較,證實為更正確且有效的中文聲母語詞聽閾檢定,可適用臨床上以中文為母語之受測者,於檢定聽閾、診斷耳疾、重聽及人工電子耳病人復健上之重大參考依據。 更精細的言語聽力學檢查,有助於病患之聽能分析、聽能復健。對於術前術後的診斷預後與學術研究分析,尤其重要。而封閉式言語聽力學檢查方法,可以有效排除不同患者間的學歷、知識背景的影響。多重檢查而具相同的測效度與信度,則可避免患者記憶與學習效應而影響測驗分數及其可靠性。

Speech audiometry has become a fundamental test for clinical otological practice. Speech audiometry is used to measure hearing threshold, to assss suprathreshold intelligibility, to monitor the progress in auditory training and lip reading, to evaluate hearing aid performance, and to predict the success of otologic surgery. However, there are few standardized consonant words lists for detailed speech audiometry testing in Mandarin. Words from elementary school textbooks are collected as a commonly used word pool by computer. Three hundreds and sixty-eight words out of 3827 words are selected. Those words are divided into 3 lists of consonant discrimination tests. Thirty ears of 15 young healthy candidates are tested to evaluate the validity and correlation of those lists. The close set list method is designed to exclude the influence of different cultural and educational backgrounds, and backgrounds, and to reduce the difficulty when testing the young children or severely hearing impaired patients with limited language development. Standard tape recording method and computer editing are the critical procedure before tests. The validity and the curve of the avergae score are constructed with this consonant discrimination test in Mandarin. The mean value with good statistic correlatioins between SRT and the 3 consonant discrimination test have proven to be highly correlated with each other and have high diagnostic value for clinical practice. Further investigation will be continued.
