嗜伊紅白血球及其毒性蛋白與慢性鼻竇炎之相關性

Correlation between Eosinophils, Eosinophil Cationic Proteins and Chronic Sinusitis

摘要

背景：過去文獻發現慢性鼻竇炎的黏膜明顯浸潤著嗜伊紅白血球(eosinophil)，而且嗜伊紅白血球及嗜伊白血球陽離子蛋血(eosinophil cationic protein, ECP)對呼吸道上皮細胞且破壞性。為瞭解鼻液及血液中嗜伊紅白血球及其毒性蛋白-ECP與慢性鼻竇炎病灶範圍之相關性，故進行此研究。
方法：本研究收集1995年至1997年間，在本科診斷為慢性鼻竇炎並揞受內視鏡鼻竇手術之患者，共41例。綜合術前鼻竇電腦斷層攝影結果和手術中之發現，依Kennedy分期方法將病灶圍分為4期。術前抽血測定血液中伊紅白血球比例、吸入性過敏血清特異性IgE抗體及ECP。並利用微吸管押吸法吸取患者鼻液，完成鼻液中ECP之測定 ; 同時取部份鼻液製成抹片，經由嗜伊紅染色法計算患者鼻液中嗜伊伊白血球比例。最後，針對各項測值進行分組及統計分析。
結果：慢性鼻竇炎病灶範圍分期與鼻液中ECP有相關性，與鼻液中嗜伊紅白血球比例(eosinophil percentage)、血液中嗜伊紅白血球比例及血液中ECP之ECP無相關性。
結論：本研究顯示標的器官(鼻部)嗜伊紅白血球釋放之毒性蛋白-ECP與慢性鼻竇炎病灶範圍分期有關。(中耳醫誌 1999;34:479-484)

BACKGROUND: We designed a prospective study to investigate the correlation of eosinophils and eosinophil cationic proteins (ECP) with the stage of chronic sinusitis.
MATERIALS AND METHODS: Forty-one patients, who had chronic sinusitis and had endoscopic sinus surgery at Provincial Hsin-Chu Hospital from 1995 to1997, were randomly selected for this study, Chronic sinusitis was staged with the Kennedy staging according to results of pre-operative sinus CT scan and operative findings. Venous blood was drawn for serum ECP, eosinophil percentage and CAP Phadiator (specific IgE antibodies to 22 common inhalant allergens) analysis. Nasal fluid was obtained by the microscution method for nasal ECP analysis. An eosin stain of nasal secretions was done to determine nasal eosinophil percentage. Pearson’s rank correlation coefficients were used to quantify the relationship between the variables of chronic sinusitis stages, the ECP values, and the percentage of eosinophils.
RESULTS:1. The stage of chronic sinusitis correlated with nasal ECP(p=0.043＜0.05) but did not correlate with the percentage of eosinophils in the nasal secretions, the percentage of eosinophils in the blood, or the ECP level in the blood.
CONCLUSION: This study demonstrated that the toxic protein (ECP)of eosinophils released in the nose (target organ) is correlated with the stage of chronic sinusitis.
