Branchial anomalies are common congenital head and neck abnormalities, but the third or fourth branchial anomalies are relatively rare. They are usually found as pyriform sinus fistulas. These patients have often undergone multiple incisions with drainage or incomplete excision that makes further treatment difficult. We experienced a case of recurrent pyriform sinus fistula presented with recurrent neck abscess and had received surgery on several previous occasions. Surgery was performed in a manner sililar to function neck dissection to achieve wide extirpation and preserve nerves, muscles as well as great vessels. The result was good and no recurrence had been noted so far. Operation wide surgical excision in a manner similar to functional neck dissection is as practival treatment method for recurrent pyriform sinus fistulas.
