鼻過敏與氣喘患者之過敏原
Allergens in Patients with Nasal Allergy and Asthma
摘要
背景：氣喘及鼻過敏是相當常見的呼吸系統疾病，查明其過敏原，不但可幫助鑑別診斷及治療，更有助於瞭解過敏原和此二疾病的關係。 方法：蒐集1997年3月至1998年1月間，至本院就診，經初步診斷為氣喘或鼻過敏症的病患共391名，分成氣喘組80名，鼻過敏症組204名，氣喘且鼻過敏症組107名，觀察並分析其接受血清多樣同時特異性抗體過敏試驗（serum multiple antigen simultaneous test, MAST）的結果。 結果：1）有特異性抗體的患者，過敏原在3組中之順序類似，以對螨類的Dermatophagoides pteronyssinus、Dermatophagoides farinae最多，佔總人數之49.9%。其中氣喘的患者，對Df、Dp過敏比例較高，且與單純鼻過敏症的患者有顯著差異。2）有過敏原的患者，96%有對家塵、螨類的抗體。3）非塵螨類過敏原的發現率，在3組間無顯著差異。4）過敏原之發現率在成年後會隨著年齡增加而遞減。5）各組內之性別不影響過敏原之發現率。 結論：在我們的調查區域，以Df，Dp過敏的比例最高，尤其年齡在15歲以下的病患，其發現率最高；其中氣喘且鼻過敏症的患者比單純鼻過敏症的患者，更易在MAST中有過敏原之發現。

BACKGROUND:Nasal allergy and asthma are two common allergic diseases which have become increasingly prevalent and severe in recent years. Clarifying the allergens helps to determine the clinical expression and relationship with the two diseases. METHODS:We analysed the results of multiple antigen simultaneous tests from 391 patients who were divided into 3 different groups based on a diagnosis of nasal allergy(204 patients), asthma(80 patients)or both(107 patients). The incidence of each allergen, MAST positive rate, immunoglobulin E antibodies, age and sex distribution were all compared. RESULTS:The most common allergens in these 3 groups were similar, and Dermato-phagoides pterynyssinus and Dermatophagoides farinae(49.9%)were the most common allergens. In addition, the group with both nasal allergy and asthma has higher MAST positive rates than the group with nasal allergy only. The peak of prevalence was found in young adults in all 3 groups and it diminished with age. However, there was no statistical difference in sex distribution between these two diseases. CONCLUSIONS:In our study, patients with both nasal allergy and asthma showed a significantly higher positive MAST rate than patients with nasal allergy only.
