摘要

唾液腺的惡性混合瘤很少見，僅占唾液腺腫瘤的2-5%，可區分成3種不同形態，包括 1) 良性混合瘤合併惡性上皮細胞（carcinoma ex pleomorphic adenoma）。2) 真正惡性混合瘤（true malignant mixed tumor，carcinosarcoma）及 3)轉移性混合瘤（benign metastasizing mixed tumor）。本院於1998年迄今分別經歷了2例惡性混合瘤病例。第1例為66歲男性以左側側咽間隙腫瘤表現的良性混合瘤合併惡性上皮細胞，經口施行腫瘤切除，並追加放射治療；第2例為轉移性混合瘤，47歲男性左側腮腺部位腫瘤10多年來多次復發並切除，這次以左側頸部淋巴結轉移來表現，故施行腮腺淺葉切除並頸部淋巴結廓清術；2病例現於門診固定追蹤，無復發情形，因均屬少見之病例，特就其臨床表現及組織形態提出報告。

Malignant mixed tumors, comprising 2-5% of salivary neoplasms, are quite rare. They can be divided into three different types, including carcinoma ex pleomorphic adenoma, true malignant mixed tumor (carcinosarcoma), and benign metastasizing mixed tumor. We present two different types of malignant mixed tumor encountered at our hospital in 1998. A 66-year-old man had carcinoma ex pleomorphic adenoma located in the left parapharyngeal space. The tumor was excised transorally and surgery was followed by radio-therapy. The second patient was a 47-year-old man diagnosed with benign metastasizing mixed tumor of the left parotid gland. A superficial parotidectomy and neck dissection were undertaken. The two cases have shown no evidence of recurrence over a follow-up period in excess of 2-years. The cornerstone of successful treatment of malignant mixed tumors is extensive surgery, with or without post-operative radiotherapy.
