頭頸癌的國際疾病分類歸碼
Problems in Coding for Head and Neck Cancers
摘要
在西方國家，頭頸癌常被認為是較罕發生的惡性腫瘤，但是在台灣地區則不然，約每10名新癌症病患就有1名屬於頭頸癌，且近年來發生率快速成長。此外，口腔和咽部的解剖位置相連，致癌因子相似，加上腫瘤容易向四處延伸，發現時往往越過數個緊臨的解剖結構，因此，目前研究頭頸癌流行病學的文獻常將不同部位的癌症歸在相同的碼上，造成頭頸癌的分類歸納及學術交流上產生混淆。其中最常被混淆者為口癌、口腔癌和口咽癌。口癌並不等於口腔癌或口咽癌；口癌、口腔癌和口咽癌各自代表一組不同部位的癌症；口癌範圍包括口腔癌、唾液腺癌和口咽癌。第3版的腫瘤之國際疾病分類部位碼，將使頭頸癌的部位歸碼更清楚。

Head and neck cancers have been wrongly considered less frequent compared with cancers from other body sites. This has led to a lack of epidemiological research in these cancers from Taiwan. In reality, 1 out of 10 new cancer patients develops in the head and neck region, and the incidence has been increasing in recent years. Moreover, because oral cavity and pharynx are contiguous to each other, it is often difficult to define the primary tumor site at the time of diagnosis when the lesion has extended beyond its origin-nal confinement. This create difficult in accurately defining and coding the various cancers originating from the head and neck region. Previous epidemiological studies, cancers from the oral cavity and pharynx are usually allocated to the same coding, result-ing in a lack of accuracy of reported data. The major tumors associated with confusion in coding of head and neck cancer include oral cancer, oral cavity cancer and oropharyngeal cancer. Oral cancer is not equal to oral cavity cancer or oropharyngeal cancer, each of which represents a distinct group of cancers. Oral cancer includes the subcategories of malignancy in the oral cavity, salivary glands, and oropharynx. The third edition of Inter-national Classification of Diseases for Oncology, with its new allocation criteria, will more exactly define the coding of head and neck cancer.
