Adult-Onset Still's Disease - Case Report
摘要
成人史提爾氏病是一種會造成不明原因發熱之疾病，它的臨床特徵除了有高燒及伴隨高燒出現之全身性紅疹外，另可出現多發性關節炎、全身淋巴病變及肝脾腫大。本科於2001年2月經歷一病例，病患為23歲年經女性，因喉嚨痛、頸部淋巴結腫大至診所求治，服藥後全身開始出現紅疹，於是轉至本科治療。安排實驗室檢查、腹部超音波及心臟超音波等檢查後，發現病人血清鐵蛋白（ferritin）與紅血球沉降率（ESR ）升高，另有肝脾腫大及多發性關節炎的症狀，故確立診斷為成人史提爾氏病。經藥物治療後，症狀顯著改善，追蹤至今逾半年，無任何復發跡象。

Adult-onset Still's disease is an illness associated with fever of unknown origin. Its clini-cal symptoms include intermittent fever which accompanies systemic skin rash, polyarthritis, systemic lymphadenopathy and hepatosplenomegaly. Otolaryngologists may overlook the diagnostic picture of this disease of its rarity. We report the case of 23-year-old female patient with chief complaints of sore throat and cervical lymph node enlargement visited our outpatient clinic in February, 2001. She has been treated with antibiotic and antipyretic at a local clinic. After taking medicine, skin rash developed and she was referred to our hospital. Ultrasonography of abdomen and heart revealed hepatosplenomegaly. Erythrocyte sedimentation rate (ESR) was increased and rheuma-toid factor and anti-nuclear antibody were negative. The findings of this patient were consistent with adult-onset Still's disease. The triad of fever, arthralgia and skin rash of the present case should serve as a reminder to clinicans.
