Verrucous Carcinoma of Vesical Diverticulum: A Case Report with the Review of Literature

摘要

膀胱疣狀癌相當少見，並且往往伴隨有血吸虫虫卵，在非血吸虫流行區，英文文獻上，僅有四例被報告，本文病例是一位74歲的中藥商，主訴解尿困難及疼痛，超音波及放射線檢查發現膀胱有一中隔和結石，手術中發現此結石中心為植物類物質，而結石下方有一疣狀腫瘤，生藥鑑定報告為參類植物，腫瘤病理報告為疣狀癌。膀胱異物慢性刺激引起疣狀癌之病理機轉可能與血吸虫虫卵相似，病人接受膀胱憩室切除及輸尿管膀胱新口吻合術近一年來情況穩定，文中並對相關文獻作一回顧。

Verrucous carcinoma is distinctly rare in the urinary bladder and often associated with positive schistosomal eggs history. There are only 4 reported cases of verrucous carcinoma of urinary blassde in the nonbilharzial epidemic area. We herein present a case with ver-rcous carcinoma of giant diverticulum where vesical foreign body with calcification was co-existent. The pathogenesis in this case may be due to chronic mechanical irritation of for-eign body like that of schistosomal eggs. The preferred treatment of verrucous carcinoma is wide surgical excision.
