We present a case of a large calculus in the urethral diverticulum which presented as triple testes in the scrotum resulting from saddle injury for more than 20 years before. The primary presenting symptoms were dysuria and intermittent pus discharge from the urethral meatus. Retrograde urethrography revealed a penile urethral diverticulum with a large stone. Diverticulectomy with stone removal and penile urethral stricture repair were performed simultaneously. The etiology, pathogenesis, management and complication are discussed.
