鼻息肉及息肉樣病變之交感神經分佈

Sympathetic Innervation in Polyps and Polypoid Changes of the Nasal Mucosa

摘要


本研究以鼻病手術切除的標本,應用甘草酸與兒茶胺形成之組織螢光法(glyoxylic catecholaminergic histofluorescence method),並採用1%中性紅(neutral red)作對照染色,探討鼻息肉及息肉樣病變的交感神經分佈,發現息肉的交感神經支配,皆集中在梗部(body)及頂端(apex)則缺乏交感神經支配,即使是血管四周亦無。而在息肉樣病變黏膜上,可見到其組織切片大致保有鼻黏膜的結構,其交感神經分佈和正常鼻黏膜之交感神經支配並為顯微鏡觀下的差別。 由結果推測:1. 就息肉的形成過程,本研究結果符合Tos等的理論,即鼻黏膜初期因水腫及發炎,使局部壓力增加,造成上皮破裂,然後形成肉芽組織的突出,穩定後,才再有上及的覆蓋形成。2. 息息肉切除時,將梗部連同息肉一起摘除,會有較好的止血效果。

In order to study the sympathetic innervation of nasal polyps and polypoid changes, the glyoxylic catecholaminergic histofluorescence method was employed in the examination of specimens taken from patients who had nasal disorders with polyps or polypoid changes. One percent neutral red was used as a conuterstain. Abundent sympathetic fibers were present around the vessels of the pedicle of nasal polyps. However, no sympathetic innervation was found in the body nad apex of the polyps. In the microscopical views of polypoid changes, there were no obvious differences between normal nasal mucosa and polypoid changes in the distribution of sympathetic innervation. Based on these results, the following conclusions were drawn: 1. We would like to advance the theory of polyp formation: Infiltration and edema in the nasal mucosa result in the rupture of the epithelium and formation of the granulations which gradually become lined with pseudostratified columuar or squamous epithelium. 2. During polypectomy, the polyp had better be removed along with the pedicle for there is abundant sympathetic innervation in the vessel which may result in good vasoconstriction and reduce bleeding.
