A 76-year-old woman with an unusual invasive meningioma growing in the parapharyngeal space is presented. It was manifested by progressive right hearing loss, mild fever and swelling of the right parapharynx. The computerized tomograpgy showed a right parapharyngeal mass and a right cerebellopontine angle mass with connection through the jugular foramen. The parapharyngeal mass was biopsied and the pathology revealed invasive meningioma. Extracranial meningioma may occur is some occasions, but rarely reported in the parapharyngeal space though it is in the vicinity of base of the skull, In this case, the tumor might arise from the right cerebellopontine angle and it extended extracranially through the jugular foramen to the parapharyngeal space, where it was found by clinical examination.
