摘要

眼腺結核是少見的病例，通常以腮腺腫瘤表現。臨床檢查極難鑑別診斷結核感染腫瘤。腮腺的Warthin氏腫瘤內，合併有結核菌感染，更是極罕見的情況。文獻上只有少數病例被報告過。成大醫院耳鼻喉科經歷一名左側腮腺腫瘤的男性病患。手術前的診斷是Warthin氏腫瘤，術後的病理檢查證實在腫瘤內有結核感染，acid-fast染色更證實了結核桿菌的存在。Warthin氏腫瘤的類淋巴基質一般認為是源於淋巴結。在這種腫瘤中發現結核浸潤，提供了進一步的證據來支持這個理論。

The lymphoid stroma of Warthin’s tumor is considered to be of lymph node origin. The finding of active tuberculous infiltration in this tumor offers further evidence to support the theory. We present hereby a case of a 66-year-old man with a left parotid mass. The preoperative diagnosis was Warthin’s tumor. The evidence of tuberculosis within Warthin’s tumor was confirmed by postoperative pathological examination. Acid-fast stain of the excised specimen revealed the existence of tubercle bacilli. The relevant literature is reviewed.
