The Audiological and ABR Aspects of Sudden Hearing Loss

突發性聽力喪失的聽力學與聽性腦幹反應

摘要


突發性聽力喪失大多是耳蝸病變引起,但有少數是因耳蝸後病變引起,如小腦橋腦角腫瘤,腦幹梗塞。若是這些耳蝸後病變遭誤診,極可能引起嚴重的後果。故對突發性聽力喪失的病人應有一有效的方法,來篩檢可能罹患耳蝸後病變,再接受進一步的檢查。本研究收集22名以突發性力喪失表現,並有詳細的聽力學及聽性腦幹反應檢查者,加以分析。結果可分為4類:第1類是聽力學特殊檢查及聽性腦幹反應均支持為耳蝸後病變者有3例;其中1例證實為腦幹梗塞,另兩例為聽神經瘤。第2類是聽力學特殊檢及聽性腦幹反應均不支持為耳蝸後病變有15例,可推想為耳蝸病變。第3及第4類為聽力學特殊檢查及聽性腦幹反應結果不一致,各兩例;其中有兩例接受神經影像學檢查,均未發現中樞聽覺徑路病變。本分析的結果支持對所有突發性聽力喪失的病人,施予聽力學特殊檢查及聽性腦幹反應作為篩檢。而對懷疑為耳蝸後病變的病人,進一步施予神經影像學檢查。

The special audiological tests and the auditory brainstem response (ABR) examination are well recognized for their usefulness in differentiat-ing cochlear from retrocochlear disorders in patients with complete audiological tests and ABR examination revealed the following results. Three had retro-cochlear disorder. Four with inconsistent results from audiological and ABR examinations lacked confirming neuroimage study to specify lesion sites. The remaining 15 cases, without retrocochlear signs in either examinations, were assumed to have cochlear disorders. One of the three retrocochlear cases turned out to be brain stem infarction victim and the other two, victims of acoustic neuroma. The results suggest that a combination of findings from special audiological test and ABR examinations are quite useful in localiz-ing lesions in patients with sudden hearing loss.
