摘要
顏面神經瘤為一種罕見之良性顏面神經腫瘤，乃源於神經鞘細胞(Schwann cell)之不正常增生所致。由於此病之臨床症狀並不特殊，而且在攝影診斷上仍然有部分不易克服之處，是以此病之誤診率依舊居高不下。長庚醫院耳鼻喉科於今年經驗一顏面予瘤病例，其臨床表現為突發性左側顏面予麻痺，並伴有漸進性左耳聽力障礙。高解像度電腦斷層掃描發現在左側中耳腔及乳突竇內有一均質腫塊，並向前延伸至岩部前上方處。經手術切除腫瘤並接受舌下－顏面神經接合術以重建顏面神經功能。特提出報告，並就其臨床症狀及診斷方面做一探討。

Facial neuroma is a kind of benign facial nerve tumor that is rarely seen. It is abnormal hyperplasia of Schwann cells. Because the signs and symptoms of this ailment are not specific and there're some hardships to be solved in diagnostic radiography, the rate of misdiagnosis still remains high. Our hospital experienced a case of extensive facial neuroma. The chief complaint was sudden onset of left complete facial palsy accompanied with left progressive sensorineural hearing loss. High resolution computed tomography showed a homogeneous mass within the left middle ear cavity and mastoid antrum, which extended to the anterioir part of the petrous bone. The patient underwent tumor excision via both transmastoid and middle fossa approches, and hypoglossal-facial nerve anastomosis for functional reconstruction.
