Massive Hemorrhage from the Innominate Artery during Tracheostomy Attempt - Case Report
摘要
氣管切開術中發生無名動脈大出血是相當罕見的併發症，而且往往有致命性的危險。成大醫院耳鼻喉部於1994年4月間經歷1名喉鈍傷之病例，因呼吸困難而施以緊急氣管切開術，於手術中遭遇無名動脈大出血，經正確的壓迫控制出血與立即開胸手術修補血管，成功地保住病患生命。由此，我們建議處理喉鈍傷的氣道問題時，若懷疑有合併大血管損傷，宜慎重並避免選擇緊急氣管切開術。

Intraoperative hemorrhage form innominate artery is very rare and often fatal complications during tracheostomy. In April 1994, we experienced a case of blunt injury to the larynx complicated by innominate artery was encountered during treacheostomy for impending airway obstruction. The patient survived from successful hemostasis by initial compression and subsequent median sternotomy with patch repair of the innominate artery. According to this experience, we suggest that tracheostomy for laryngeal trauma should be hold if great vessel injury is suspected.
