耳蝸性聽力障礙之歪成分耳聲傳射

Distortion Product Otoacoustic Emission (DPOAE) in Cochlear Hearing Loss

摘要


耳蝸性聽力障礙病人16例,年齡由21到65歲,男性7人,女性9人,其中5例是單耳聽力障礙,11例是兩耳聽力障礙,總共26個感音神經性聽力障礙耳為研究對象。歪成分耳聲傳射檢查儀器是採用GSI 60 DPOAE System (Grason-Stadler廠),檢查在靜室中進行。刺激音是2個不同頻率的連續純音F1和F2(F2﹥F1,F2/F1=1.2),刺激音頻率由500Hz-6031Hz,刺激音壓強度保持一定(L1=L2),音壓強度分別為60 dB,65dB,70dB,75dB,和80dB SPL;每次檢查在11個測定點記錄歪成分耳聲傳射(2F1-F2,HZ)之大小。本研究結果顯示,無論是何種病因,耳蝸性感音神經性聽障耳之耳聲傳射圖和純音聽力圖之頻率特性均非常符合,尤其刺激音壓70dB SPL時兩圖形最相似,且耳蝸性感音神經性聽障耳之歪成分耳聲傳射值和歪成分耳聲傳射圖之再現性良好。歪成分耳聲傳射檢查和純音檢查一樣具頻率特性,故臨床上可應用於中耳功能正常之耳蝸性病變之診斷或耳蝸聽覺功能之監控。

Distortion product otoacoustic emission (DPOAE) was investigated in 5 cases with unilateral cochlear hearing loss and 11 cases with bilateral cochlear hearing loss. DPOAE was recorded by a GSI 60 DPOAE System (rev.4.0). The F2/F1 ratio was fixed at 1.2 and DP level at 2F1-F2 (Hz) was measured at 11 points of geometric mean of F1 and F2 between 531 Hz and 5500 Hz. The stimulus intensity was from 60 dB to 80 dB SPL. The distortion product-gram (DP-Gram) was drawn from the DP levels at 11 individual points of geometric mean of F1 and F2. The results showed: In spite of different etiologies, the excellent correspondence with frequency specificity was found between pure tone audiogram and distortion product-gram. The short-term reproducibility of the configuration of DP-gram and DP levels was excellent in the ears with cochlear hearing loss. DPOAE could be a very useful clinical monitor for objective investigation of the frequency specific cochlear function in the ears with cochlear hearing loss.
