The hyoid syndrome is generally unknown and under-diagnosed. This paper reports 7 patients with pain from the greater hyoid cornu. The main presenting symptom in each case was lateral neck pain which was usually initiated and intensified by swallowing or movement of the neck to the affected side. Physical examination of the neck revealed that palpation over the tip of the affected greater cornu of the hyoid bone could elicite severe pain. CT scan image of the neck demonstrated that the affected greater cornu was located closer than normal to the internal carotid artery or had an abnormal lateral curvature. Treatment with excision of greater hyoid cornu under general anesthesia was effective in all cases. There was no complications and the neck pain was either signicantly decreased or completely relieved. The purpose of this paper is to increase greater recognition on the symptom complex of the hyoid syndrome, so as to provide more appropriate management and to prevent unnecessary investigation and surgery.
