Nearly 60-80% of benign tumors in the parotid gland is mixed tumor. Simple enucleation carries a high recurrent rate (25-50%) due to incomplete removal of the tumors. Approximately 2-9% of all recurrent mixed tumor undergo malignant transformation. The malignant transformation possesses a correlation between the duration and frequency of the recurrence. The most common malignant change of mixed tumor is carcinoma ex pleomorphic adenoma. In the literature, there is only one case report with recurrent mixed tumor following malignant transformation to the adenoid cystic carcinoma in the parotid gland. We present a case of recurrent mixed tumor of right parotid gland which has been treated by surgical resection for three times. But unfortunately, the masses recurred again, and she received radical parotidectomy with facial scarification later. The pathological report was adenoid cystic carcinoma, cribriform type. Yet, after that the patient received post-operative radio-therapy, there been no evidence of recurrence till now.
