以聲反射鼻量計評估正常人與鼻塞患者鼻腔通暢度的差異

Patency of Normal and Obstructive Nasal Cavities Evaluated by Acoustic Rhinometry

摘要


聲反射鼻量計可方便地用以估正常人與鼻塞患者鼻腔的通暢度,利用此鼻量計測得的曲線圖形可分為三種,即升W形、降W形及V形。不論正常組或鼻塞組,最常出現的曲是降W形。本研究在較嚴格的實驗控制下,發現正常組與鼻塞組由聲反射鼻量計測得的數據有所差異,正常人單側鼻腔最小截面積平均約0.81 cm^2,當一側鼻腔最小截面積在0.57 cm^2以下時,可說病患有客觀的鼻塞現象。而在設定鼻腔深度7 cm時,正常人單側鼻腔容積平均約10 mL。研究中發現,83%(83/100)的正常人與94%(32/34)的鼻塞者鼻腔,其最小截面積發生在下鼻甲前端,由此推測,大部分國人的鼻閥位置在下鼻甲前端。

Acoustic rhinometry could be conveniently used to evaluate the nasal patency. In our study, three patterns of the acquired reflected curves were noted as follows: ascending W, descending W and V shape. The descending W pateern was found most frequently in normal and obstructive nasal cavities. There were 83% (83/100) of normal nasal cavities and 94% (32/34) of the obstructive ones had the minimal cross-sectional areas occurring near the anterior end of inferior turbinate. The means of the minimal cross-sectional area and volume of unilateral normal nasal cavity were 0.81 cm^2 and 10 mL respectively. We concluded that an objective nasal obstruction be noted for the patient when the minimal cross-sectional area of one side of his/her nasal cavity was found to be below the value of 0.57 cm^2.
