摘要

多發性對稱性脂瘤病（multiple symmetric lipomatosis）又稱Madelung’s disease。自1846年Brodie首例病例報告以來，迄今文獻中約只有200例。此病特微為在頸背部、鎖骨上部、及上肢近端處出多發性且不具被膜之對稱性脂肪堆積。病人除外觀特殊外，臨床上也因其有會壓迫到喉部、緃膈腔、氣管等而有相關之症狀。少數病人亦會合併有肝臟病變、糖尿病、痛風及高脂血症等。據統計有60%~90%的病人有酗酒的習慣。
本科於1995年7月經歷1病例，主訴為持續兩年頭部對稱巨大腫塊，經手術切除及病理報告證實為脂肪瘤，即為所謂的Madelung’s disease，由於此病較罕見，特此提出報告供讀者參考。

Multiple symmetric lipomatosis (Madelung’s disease) is a rare disease which appears mostly in the neck and the proximal portions of upper extremities. It is more frequent seen in countries around the Mediterranean Sea, and occurs predom-inantly in males who are alcoholic.
In July 1995, we encountered a 52 y/o male patient who had suffered form diffuse symmetric swelling of the whole neck area since 2 years ago. The cytopathologic study of the fine needle aspiration revealed lipoma. Under the impression of Madelung’s disease, he received a successful surgical excision. He was regularly followed up at our out patient clinic. No evidence of recurrence was noted.
